2. House-Keeping Items and Beginning of the Year Notifications

a)  Parking/access at the SOESD

b)  Work Assignment Acknowledgement

c) Absences:  Please contact Dawn when you are absent because of illness, doctor appointments or have an unexpected absence (discretionary leave) and enter into Frontline.  Flex leave requests are approved by Agnes prior to your leave.  

 

d)  Yearly Work Calendar: 

All agency staff are required to complete a working calendar.  Finalized calendars will be sent to the business office in September. The business office realizes that calendars may change throughout the year. Those changes need to be emailed to Dawn.  If your calendar changes during the year, revisions will be made, initialed, and Dawn will submit the revised calendar to the business office.  

*Complete a draft calendar and submit to Dawn by Sept 3rd.

*Dawn will enter the draft into the electronic calendar.

*The final calendar needs to be initialed by the service provider and by Agnes.  

 

e) Insurance-Open enrollment:   Please check the website for instructions on whether you need to enroll.

g) In-District Travel Reimbursement: Travel Reimbursement is completed on our new travel page: https://travel.soesd.k12.or.us/login.  

 

h) Employee Request to Attend Inservice/Conference/ Workshop:  A sample copy of the document is at the bottom of agenda.  Yellow highlights indicate the portion completed by Attendee.  All in-person requests must be turned in to Dawn before the workshop.

 

i) Out of Area Travel Reimbursement:  This form should be completed prior to or shortly after returning from a conference or workshop.  The form is located on your SOESD Business Forms.

 

j) Courier Services – Please use complete name of the entity on Courier mailings.  See courier schedule link  on for names.  

Ex: SOESD Medford

 

k)  Phone and Email Etiquette 

Work voicemails and emails (outgoing messages) need to be set up by Sept 3rd.  Incoming phone messages and E-mails need to be checked at least once every 2 working days.  The SOESD does not allow “Confidentiality” disclaimers on our outgoing e-mail messages.

 

b)  form needs to be returned to Dawn at the end of this meeting.  




 

d) Work calendars are due to Dawn by Sept 3rd

f) These trainings must be completed by September 30, 2021.

g)  Please see Dawn if you need Travel Log documentation training.

 

h) Attached flyer for the conference showing content must be submitted with the request. Duty request time must be entered into Frontline as an absence.



 

i)  If you want help completing the Out of Area Travel Reimbursement you can call Dawn and set a time to go through the procedure.

3. SOESD Procedures for district/student referrals

a) “Request for Student Services” form 

 

b) Purging Policy (refer to procedural document)

 

c) Student Report Archiving to the G-Drive shared Drive ASD or Psych Reports - Please, refer to instructional “cheat sheet”.  Evaluation reports must be written on SOESD LetterHead (page 1), with pages numbered, and with name/title and signature of evaluator on the last page.

 

d)  Use of Service Log and Time Sheet - This form is generally used for service that falls outside of your regular FTE.

c) Reports by service providers should be saved to the G-Drive at the time of completion.
